In this chapter

This chapter asks what a UAE cosmetic clinic can afford to pay for an enquiry and a new patient, and what published evidence exists to test that number. No measured UAE dataset of clinic cost per click, cost per lead, conversion, no-show or patient lifetime value was found; the UAE cost figures that exist are agency claims graded Estimate, such as AED 300 to 800 per qualified cosmetic surgery consultation in Dubai. The published benchmarks are mostly US, such as a US median search cost per lead of USD 39.25 for beauty and personal care, so the chapter then sets out ONS planning models with hypothetical inputs that a clinic can replace with its own results, allowing enough time for patients to book, attend and decide on treatment (WordStream by LocaliQ, 2025).

In brief

US med spas averaged USD 527 per patient visit, USD 1,398,833 revenue per med spa, 245 patient visits per month and 73% repeat patients in AmSpa's 2024 report.

North American med spa data from Zenoti show a 4% no-show rate and 14% cancellation rate in 2025; an undated UAE estimate put outpatient no-shows at 37%. The figures measure different things and conflict.

In 145,000+ US aesthetics enquiries, 50.1% converted to a scheduled appointment, and caller procrastination explained 41.7% of non-bookings (Liine, Jan to Dec 2023; Liine, 2023).

Firms contacting a web lead within an hour were nearly 7 times as likely to qualify it, in a 2011 US cross-industry study (SmartCompany).

US search medians: beauty and personal care CPC USD 4.62 and CPL USD 39.25; physicians and surgeons CPC USD 4.76 and CPL USD 40.04 (WordStream by LocaliQ, 2025). US Meta lead campaigns cost more per lead: USD 51.42 and USD 47.47 (WordStream, 2024).

Dubai CPC claims such as AED 25 for "rhinoplasty dubai" and AED 6 to 12 for non-surgical terms come from an agency page with no method or date.

The results of the three ONS service models can be recalculated from their stated inputs: the same AED 18,000 media budget gives contribution after acquisition of AED 24,522 (skin course), AED 3,926 (injectables) and AED 40,680 (surgery).

Break-even media cost per valid enquiry is AED 212.61, AED 162.41 and AED 733.50 for the three models, against modelled CPLs of AED 90.00, AED 133.33 and AED 225.00.

The injectables model is the most fragile: a 20% rise in CPC, a 10-point fall in booking or a 10% fall in receipt each turns its contribution negative.

1. How to use published benchmarks

Every benchmark in this chapter carries its geography and period. Most are US or global; none is a UAE cosmetic clinic dataset, and the research notes record no public UAE dataset of clinic CPC, CPM, CPL, conversion, no-show or lifetime value (Enrich Labs, 2025 to 2026; Admoon agency site). US figures are useful for asking whether a funnel stage looks plausible and whether a metric definition needs checking. They cannot set a Dubai bid, an Abu Dhabi budget or a Sharjah target. No exchange rate is applied in this chapter: USD figures stay in USD and AED figures stay in AED, because converting a US median into dirhams would make it look like a UAE number.

A platform "conversion" or "lead" is whatever the advertiser told the platform to count; it is not a valid enquiry, an attended consultation or a paid treatment. The ONS models keep these stages apart, using the funnel definitions in Chapter 16.

2. Clinic economics benchmarks (US and North America)

The American Med Spa Association (AmSpa) 2024 Medical Spa State of the Industry Report, recapped on 6 November 2024, gives an average spend per patient visit of USD 527, average revenue per med spa of USD 1,398,833, 73% repeat patients, 89% female patients and 245 patient visits per month. A second compilation of the same AmSpa recap reports 10,488 US med spa locations in 2023 and labels the figures as 2023 data. One secondary agency page describes USD 245 as "monthly spend per patient"; the verified AmSpa recap gives 245 as patient visits per month, and this book uses that reading (Web Tonic, 2023 to 2026). AmSpa's own statistics page puts US medical aesthetics revenue at "$17B+", growing by more than USD 1 billion a year, report year not stated (American Med Spa Association, 2026).

Zenoti's 2026 benchmark report on its North American med spa customers gives a median average ticket of USD 216 (75th percentile USD 346, 90th percentile USD 484) and median revenue per location of USD 1.86m for 2025. The two sources measure different populations, so their revenue figures are not interchangeable (American Med Spa Association, 2022; Zenoti, 2025).

Two lifetime value claims conflict: USD 7,200 to 12,000 in one agency compilation and up to USD 4,775 over 18 months in another, neither with a published method (Foundry CRO, 2026; Prospyr, 2026). AmSpa's 2017 report put repeat patients at 70% of visits (American Med Spa Association). AmSpa's own lifetime value figure is not on its public pages (American Med Spa Association, 2022). A UAE clinic should not import either figure into its affordability arithmetic.

What it means for marketersRepeat patients dominate US med spa activity (73% in AmSpa 2024) and new guest visits fell 11% in Zenoti's 2025 North American data. If the same pattern holds in a UAE clinic, the value of a new patient depends heavily on whether the clinic keeps them, which is why the ONS models below report first-treatment contribution only and leave retention as a separate, clinic-measured input.

3. Funnel benchmarks: response, booking, attendance and treatment

Lead response

The most cited evidence on response speed is old and cross-industry. An audit of 2,241 US companies found 37% responded to a web lead within an hour; average response time among those responding within 30 days was 42 hours (SmartCompany, 2011). A separate study of 1.25 million leads found firms contacting within an hour were nearly 7 times as likely to qualify the lead as those trying even an hour later, and over 60 times as likely as firms waiting 24 hours or more (US, 2011) (SmartCompany). The older Lead Response Management Study reported that contacting within 5 minutes rather than 30 minutes gave about 100 times higher odds of contact and about 21 times higher odds of qualifying (US, 2007, no healthcare data) (Callingly).

Aesthetics-specific evidence is weaker: in one US mystery shop, 40% of aesthetic practice test leads sent over a weekend had no reply by Monday (Etna Interactive, 2024). In UAE resident surveys, 85% would prefer to use WhatsApp to interact with companies and 87% want a real person rather than a chatbot (YouGov for Zbooni, n=1,000, 2024); 88% say WhatsApp is the easiest way to get quick, accurate business responses (Communicate Online).

Consultations leading to paid treatment50% to 70%US, vendor compilation, 2026Prospyr [Estimate]

Three conflicts matter. First, the two vendor compilations disagree on channel booking rates: Google leads book at 25% to 40% against social at 10% to 22% in one, and Google at 40% to 60% against Meta at 25% to 40% in the other (Prospyr, 2026; Web Tonic, 2025 to 2026). Second, the Liine enquiry-to-appointment rate of 50.1% sits far above the "about 1 in 9" claim and the 8% to 12% med spa lead-to-booking range, partly because the denominators differ (all new patient enquiries against all leads). Third, no-show figures range from 4% in Zenoti's North American med spa data to 15% to 25% in an agency compilation and 37% for UAE outpatients in an undated estimate. The UAE figure covers all outpatient appointments, not cosmetic consultations, and no public consultation show rate for aesthetic clinics was found in the US or the UAE (Dubai Healthcare City Authority).

Vendor claims on cutting no-shows are not independent: 14% after 12 months on one booking platform, 25% to 40% from a three-touch SMS sequence and "up to 75%" claimed by Okadoc in 2020 (Boulevard newsroom, 2022 to 2023; Web Tonic, 2026).

ONS reading The most defensible published anchors for a clinic funnel model are an enquiry-to-booking rate near the Liine figure and a consultation-to-treatment rate near the AmSpa guidance, and both are US figures. ONS uses them only to check that a clinic's own rates are not implausible. Before setting its budget, a UAE clinic needs to measure booking, attendance and treatment rates for each group of enquiries.

4. Paid media cost benchmarks by geography

Search and Meta

WordStream by LocaliQ's US search medians for April 2025 to March 2026 put beauty and personal care at a CPL of USD 39.25 and physicians and surgeons at USD 40.04. These are separately calculated medians, so the median CPC divided by the median conversion rate does not reproduce the median CPL (WordStream by LocaliQ, 2025). LocaliQ's 2026 page, built on customer campaigns across Google Ads and Microsoft Ads with geography not stated, gives health and fitness a CPL of USD 67.36 and all industries USD 66.69. US Meta lead campaigns (April 2024 to June 2025) cost USD 51.42 per lead for beauty and USD 47.47 for physicians and surgeons; a secondary page quoting the 2026 edition gives USD 50.91 and USD 32.14, graded Estimate because the primary page was not read (WordStream; Web Tonic agency blog citing LocaliQ/WordStream 2026 Facebook ads benchmarks). For the UAE, Superads' tool dataset shows an all-industry Facebook CPM averaging about USD 15.8 between July 2025 and July 2026, ranging from USD 8.92 (May 2026) to USD 23.52 (November 2025) and about 23% below the global CPM of about USD 20.6, with no healthcare split.

5. UAE cost claims and regional examples (Estimate)

The UAE cost figures in the notes are agency claims without method or dates, and they are presented here only so a clinic can recognise them when it hears them. One agency site claims Dubai Google Ads CPCs of AED 12 to 25 for hair transplant, AED 15 to 30 for facial cosmetic surgery, AED 10 to 20 for body contouring and AED 6 to 12 for non-surgical terms, with keyword examples of AED 25 for "rhinoplasty dubai" and AED 28 for "facelift dubai". The same source claims AED 300 to 800 per qualified cosmetic surgery consultation, AED 150 to 500 per laser and aesthetic consultation lead and AED 80 to 250 per qualified dermatology consultation (Admoon agency site). A separate cost guide, quoted by another agency, puts healthcare Google Ads CPCs in Dubai at AED 5 to 15 (Enrich Labs, 2025 to 2026).

The two US cosmetic surgery CPL claims, USD 134.29 and about USD 610, differ by a factor of more than four and neither names a dataset (Prospyr, 2026; Foundry CRO, 2026). Two Dubai agency case headlines, an 83.4% revenue uplift for SKIN111 in 2023 and +214% secured consultations for an unnamed surgery centre, give no baseline or period (Econsultancy; Web Tonic). Snap and Publicis report a Snapchat median cost per action 14% below other social platforms in MENA, in a commissioned study naming no healthcare advertisers (Communicate Online, 2026). WARC's severe Gulf conflict scenario takes GCC ad spend growth from about 11.7% to about -0.2% in 2026, with no UAE or clinic split (AdNews).

ONS reading The agency claims are not evidence of what a UAE clinic will pay, but they are useful for one check. The hypothetical CPCs in the ONS models below, AED 9 and AED 12 for non-surgical services and AED 18 for surgery, fall inside the claimed Dubai ranges of AED 6 to 12 for non-surgical terms and AED 15 to 30 for facial cosmetic surgery. That makes the models plausible starting points, not forecasts. A clinic should replace every input with its own account data as soon as it has a mature cohort.

No public data. No measured UAE dataset was found for cosmetic clinic CPC, CPM, CPL, enquiry-to-booking, consultation show rate, consultation-to-treatment rate or patient lifetime value, and no split by Dubai, Abu Dhabi, Sharjah or the other emirates exists for any of them (Enrich Labs, 2025 to 2026; Admoon agency site). A future edition should collect anonymised cohort data from consenting UAE clinics by emirate and service family.

6. ONS planning models D01 to D03: three services, one media budget

The three models below are ONS illustrations with hypothetical inputs. They are not UAE campaign forecasts, market fee benchmarks or observations of any clinic's account. Each gives the same monthly media spend to a different service family so that the effect of price, delivery cost and conversion on acquisition economics can be seen side by side. The money inputs, CPCs and click-to-enquiry rates are those of the ONS draft. The draft text states only the composite outcome of the stages after the enquiry; the stage rates shown here are an ONS decomposition chosen to reproduce the draft's outputs exactly, and any other set of stage rates with the same product would give identical results.

The eligibility rate is an observed constraint set by clinical assessment; the model does not treat it as a commercial target. Expected paid starts may be fractional; actual patients are whole people. Contribution excludes fixed clinic overhead and is not net profit.

Revenue multiples can mislead

Revenue divided by media spend is 4.49x for D01 (80,870.40 / 18,000), 3.10x for D02 (55,851.12 / 18,000) and 8.98x for D03 (161,700 / 18,000). D02's 3.10x sits inside the 2.5 to 4x blended healthcare and wellness ROAS one agency calls a UAE benchmark, yet its contribution after acquisition is only AED 3,926 because half of each receipt goes on delivery cost.

ONS reading The models show why the same media budget can produce very different economics. A larger receipt does not guarantee the best result, and a weaker result does not prove a service is unviable: D02 is close to break-even because of its delivery cost and conversion assumptions, not because injectables are unprofitable in general. The answer for any clinic depends on its actual delivery cost, conversion, capacity and patient choice. ONS recommends that a clinic sets a lead or patient target only after building this chain from its own mature cohorts.

7. ONS planning model D04: break-even cost per enquiry

Break-even media CPL is the media cost per valid enquiry at which contribution after acquisition is zero, before fixed overhead. With enquiry volume held constant:

Break-even media CPL = (contribution per paid start x enquiry-to-paid-start rate) - (other acquisition cost / valid enquiries)

Dividing by the click-to-enquiry rate gives the matching break-even CPC; multiplying by enquiries gives the break-even media spend, which equals gross contribution less other acquisition cost. The maximum fully loaded CAC at break-even is simply the contribution per paid start.

Check for D01: at a CPL of AED 212.61, media spend is 212.61 x 200 = AED 42,522; adding other cost of AED 6,000 gives AED 48,522, equal to gross contribution, so contribution after acquisition is zero. The break-even CPL is a zero-contribution boundary. A commercial target should leave room for fixed costs, uncertainty and profit, so it should sit below break-even, not at it. D02's modelled CPL already uses 82% of its break-even, leaving little room for error.

For comparison only, the agency-claimed Dubai cost of AED 300 to 800 per qualified cosmetic surgery consultation is measured at a later stage than the D03 valid enquiry. In D03 the media cost per attended consultation is 18,000 / 23.1 = AED 779. A clinic comparing its own numbers with any external claim must first confirm which stage the claim measures.

Source: ONS planning model D05, hypothetical inputs

ONS reading Conversion rates and delivery cost should be tested independently because they behave differently in practice. A booking improvement may have little value if the clinic cannot accommodate follow-up. A higher price can improve revenue per case but reduce the number of appropriate patients who choose to proceed. The sensitivity table is useful only when these operational relationships are considered alongside the arithmetic, and a model as close to break-even as D02 should not be scaled until its own measured rates are known.

9. ONS planning model D06: the capacity ceiling

A media plan should be limited to what the clinic can deliver responsibly. The model converts expected demand into required consultation and treatment slots and compares them with available capacity. A campaign that sells more courses than the clinic can schedule creates obligations, not immediate profit.

Media needed to fill available consultation slots = media x (available slots / modelled attended consultations)

Uncapped contribution (D01, D03 base)24,52240,680[ONS]

When consultation capacity is limited, spending the full budget generates enquiries from people the clinic cannot see: in D01, AED 3,937 of the AED 18,000 (18,000 - 14,063) produces no attended consultation. The same check applies downstream. If the D01 course were six sessions, 44.928 paid starts would need 44.928 x 6 = 270 treatment sessions over the life of the courses, and in D03, 6.468 starts need seven theatre slots plus follow-up. Six sessions and the slot counts are hypothetical.

What it means for marketersCancellation rates in Zenoti's North American data were 37% for appointments rebooked once and 4% for those rebooked two or more times, and staff taking a message accounted for 15.6% of non-booked US aesthetics leads. Capacity is not only rooms and clinicians; it is also reception time to answer, book and rebook. A clinic should count both before raising the media budget.

10. ONS planning models D07 to D10: budget envelopes

The draft's illustrative monthly envelopes separate media from the implementation and measurement work that makes media usable. They are planning illustrations only, not market fee benchmarks, quotations or recommended minimum spends for every clinic. Totals are direct sums.

Arithmetic: 18,000 + 6,000 + 3,000 + 3,000 = 30,000; 40,000 + 10,000 + 5,000 + 5,000 = 60,000; 75,000 + 18,000 + 8,000 + 9,000 = 110,000. All three totals reproduce.

D07 and D01 to D03 are not fully consistent: the service models load AED 6,000 of other acquisition cost, while D07 carries AED 12,000 of non-media cost around the same AED 18,000 of media. If one service carried the whole D07 envelope, contribution after acquisition would be 48,522 - 30,000 = AED 18,522 for the skin course, 27,926 - 30,000 = AED -2,074 for injectables and 64,680 - 30,000 = AED 34,680 for surgery, with fully loaded CACs of AED 668, AED 859 and AED 4,638. These figures equal the "other acquisition cost +AED 6,000" row of the sensitivity table.

ONS planning model D10: paid starts needed to cover each envelope

Dividing each envelope by contribution per paid start gives the starts needed to cover it, rounded up. For D07 (AED 30,000): skin course 30,000 / 1,080 = 27.8, so 28; injectables 30,000 / 800 = 37.5, so 38; surgery 30,000 / 10,000 = 3. For D08 (AED 60,000): 55.6, so 56; 75; 6. For D09 (AED 110,000): 101.9, so 102; 137.5, so 138; 11.

D10 is a break-even test for the whole envelope if a single service carried it, before fixed overhead. A growing portfolio or multi-branch group would mix services, and each branch needs its own capacity check (D06) before the media line is raised.

Paid media allocation and the agency commercial model

ONS reading Start with service and channel combinations that have a clear destination page, current approval and available capacity, keep a small explicit testing allowance, and do not assign a fixed percentage to each emirate on population alone. A clinic with weak pages or no reliable appointment attribution may need implementation work before a larger media budget is productive. An agency scope should define deliverables, branches, service families, content review and reporting cadence, keep media separate from fees, and base any performance incentive on auditable measures that do not encourage unnecessary treatment or distort clinical suitability decisions. Pause rules are set out in Chapter 19.

Gaps in the evidence

No public data. No UAE or emirate-level dataset was found for clinic CPC, CPL, enquiry-to-booking, consultation show rate, consultation-to-treatment, cancellations, retention or lifetime value; the only UAE cost figures are undated agency claims. No public consultation show rate for aesthetic clinics exists in the US or the UAE, and the only UAE no-show figure is an undated all-outpatient estimate (Dubai Healthcare City Authority). AmSpa's lifetime value figure is behind its paid report. Where to look next: anonymised cohort exports from consenting clinics in Dubai, Abu Dhabi, Sharjah and the northern emirates; booking platform aggregates; and the full AmSpa and Zenoti reports. A future edition should publish UAE medians by service family with fixed definitions for valid enquiry, attended consultation and paid start.

Model inputs are hypothetical. The ONS planning models D01 to D10 use invented inputs for illustration. The draft text does not publish its stage rates after the enquiry; the rates shown were chosen by ONS to reproduce the draft's outputs and are not observations. No UAE delivery cost data by service was found, so the receipts and delivery costs are not benchmarks.
